Burial Preferences

The purpose of the form below is to ensure that, in the event of your death, we are able to provide the services you wish, even if your next of kin are unaware of your wishes.

Completion of the form in whole or in part is entirely optional. If you are not ready to complete the entire form it would be helpful to have your English and Hebrew names, your date of birth, and contact names and numbers of next of kin or other close family members.

Whilst the NLJC, its Council and its members cannot be bound by requests made, nevertheless the information will be taken to represent your wishes and will be followed as far as possible. The information is handled in the strictest confidence. Sealed copies of the form are kept by members of the Burial Group and will be opened only in the event of your death.

If in the future your circumstances change you should let any member of the Burial Group know. It would also be sensible to supply your next of kin with a copy of the completed form.
